TransAsia Airways crash report lays blame at pilot error

The official report into last year’s TransAsia Airways crash has levelled blame at the pilots who failed to respond correctly to a mid-air engine failure.
Expanding on findings released in an earlier preliminary report, Taiwanese investigators said pilots mistakenly shut down the working engine when the other engine failed.
They realized this error too late to prevent it crashing into a river killing 43 passengers.
"The flight crew did not perform the documented abnormal and emergency procedures to identify the failure and implement the required corrective actions," the report said.
The report also made safety recommendations aimed aircraft maker ATR and engine manufacturer Pratt & Whitney to conduct more detailed risk assessments.
The airline was also ordered to ‘conduct a thorough review’ of flight crew training and improve oversight of safety, training and pilot performance.
